Often people attend Mass at a parish for months or sometimes years, but never officially register. Registering at St. Francis of Assisi Catholic Church is the expression of your commitment to being part of our parish community. Being a registered parishioner also makes the process much simpler when it is time for
- receiving Sacraments,
- registering for Childern’s Faith Formation,
- becoming a Sponsor or Godparent,
- Marriage preparation andWeddings, and
- planning Funerals.

Complete the form, save it, and then send it back to St. Francis of Assisi Catholic Church parish office by:
- e-mail, or
- USPS mail to 18825 Fuller Heights Road, Triangle, VA 22172, Attn: Lina Morales; or
- bring to the parish office, or
- drop it in our catch-all slot in the vestibule of the Church
